Long-term care programs help to ensure a positive quality of life for seniors and assist in delaying or preventing premature institutionalization, including:

Senior Assisted Living Program

The Aging & Community Centers Division maintains a complete list of licensed assisted living programs (ALPs) in Charles County and can provide seniors and their families with helpful tips in selecting an ALP. Assisted living is typically designed to provide housing and supportive services for frail seniors who need daily assistance and, therefore, are at risk of nursing home placement.

For additional information, please call 301-934-0133 or 301-870-3388 ext. 5133.

Senior Care & In-Home Care

Programs are designed to provide access and coordination of services to frail, health-impaired elderly in an effort to allow them to remain in their homes. Financial assistance may be available to eligible clients for services such as in-home care (bathing, light housekeeping, meal preparation), medications, personal care supplies, and home monitoring.

For additional information, please call 301-934-0128 or 301-870-3388, ext. 5128.

Family Caregivers Support Program

As a participating agency in the National Family Caregivers Support Program, the Aging Division offers comprehensive services and support to family members who are caring for an elderly adult relative. Information & Assistance, Respite, Caregiver Training, Support Groups, and Supplemental Services are included in this program. Support Groups meet the last Wednesday of each month at 2:00 P.M., at the Richard R. Clark Senior Center in La Plata, Maryland.

For additional information, please call 301-934-0128 or 301-870-3388, ext 5128.

For information call 301-934-0133 or 301-870-3388, ext. 5133.

Medicaid Waiver for Older Adults

The Medicaid Waiver is a state medical assistance program that may provide financial assistance for community-based care to persons who would otherwise require a nursing home. Qualified individuals must meet both medical and financial criteria to receive a waiver. Waivers can be used for either assisted living or in-home care.

For information call 301-934-9305 or 301-870-3388, ext. 5145.
