Senior Care Program
The Senior Care Program, provided through the cooperative efforts of the Charles County Department of Community Services Aging & Community Centers Division, the Charles County Department of Social Services, and the Charles County Health Department, is designed to help frail persons age 65 and older who are at risk of entering a nursing home to remain in the community.
Through the Senior Care Program, clients receive a comprehensive, in-home assessment completed by a registered nurse from the Charles County Health Department Adult Evaluation and Review Services (AERS). Following this assessment, a Joint Care Planning team, comprised of representatives from each of the three agencies, develops a plan of care for each individual client which is designed to link individuals with all suitable and available resources and services. Clients who meet the following Senior Care eligibility criteria may receive case management services which include facilitating access to resources, care coordination, assessing needs, and follow-up as required.
- 65 years of age or older Moderately or severely disabled as determined by AERS guidelines Current resident of Charles County
- In need of assistance with activities of daily living, such as bathing, dressing, and meal preparation
